2 May 2019 (Thursday) - BTLP-TACT


I did a BTLP-TACT exercise this evening.

I was presented with two cases

21378 – a ninety-year old male undergoing vascular surgery requiring group & save
26846 – a twenty-year old male with sickle cell disease requiring eight units of blood within the hour for exchange transfusion

21378 was A Negative with positive antibody screen. I requested IAT and enzyme panels. The IAT and enzyme panels were positive in cells 1 2 and 3 - it  looked like anti-D but Cw could not be excluded

26846 was AB with what I assume was meant to be a weak D. I reported it as AB indeterminate and decided to give Rh Negative blood.
The antibody screen was negative.

·         There was no O Neg or A Neg HbS Neg irradiated blood available.
·         There was one A Neg HbS Neg available.
·         There was two O Neg HbS Neg available.
·         There was no O Neg Emergency HbS Neg available.

So… whatever I did here would be wrong.
I gave eight O Neg in view of the urgency. I don’t think the software registered this. And I resigned myself to getting a red light.

I got a red light. I’d like to know what I should have given…
