This picture was posted in one of the haematology-related Facebook groups I follow with the comment “What is the diagnosis”. With absolutely no clinical or demographic information whatsoever there must be an element of guesswork here. But the hypersegmented neutrophils immediately suggest a megaloblastic process – I’m sure I can see a Howell-Jolly body in there. However the red cells show a dimorphic picture; is there a concurrent iron deficiency? And is that a blast cell as seven o’clock?

I was rather intrigued by some of the comments on the Facebook group. Do people *really* think this is C.M.L., and what exactly do they mean by “Wbc abnormality”?
