9 June 2025 (Monday) - XbarM Analysis


The XbarM alarm is in theory an absolutely brilliant idea. Given that the population with which we are dealing remains constant then the average result of any given blood parameter will also remain constant. And so any serious change in the average of the measurements of that parameter will tell us (me!) that there’s something causing that result to change.

However there is one big assumption in there… if we take the day’s workload and compare it to yesterday’s or tomorrow’s then the population with which we are dealing probably does remain constant. However we aren’t comparing on a daily basis. We are comparing batches of twenty patients. And bearing in mind that the various clinics batch up their work and sent it to us in job lots, XbarM analysis effectively ends up comparing twenty ante-natal patients with twenty oncology patients.
And so the alarm goes off.

Well, the answer was “severe burns”. I got it right, but bearing in mind I’ve seen precisely one Cabot ring in forty-four years of this game, would a better answer be “pretty much every malady known to medical science”?

I’m getting too old and too cynical for this game…

Here’s an interesting article from the Sysmex Academy. The XbarM control program.
In theory it is a brilliant idea… the average result of all the samples going through the lab should remain unchanged over time.
It probably does… from one week to the next. However all the time patient samples from specific clinics arrive together there are going to be biases. And so XbarM is of arguable relevance. The trouble is that because the analyser comes with this tool, people will use it. With no regard to how it is being used. And so the old maxim comes into play – “if you look for trouble, you will find it”.

9 August 2021 (Monday) - Sysmex Academy Update

The Sysmex Academy sent out their newsletter today. You can see it by clicking here. To their credit they are running a series of webinars outlining how their technology works…

However…

They are running webinars. These run at a fixed time and are only open to a fixed number of people. I looked at one or two. I couldn’t get out of work for an hour at the busiest time to see the webinar, and even if I could the things were fully booked and there was a waiting list.

Why, oh why, do people still do the “webinar” thing? Why not just release a video MP3 file that anyone can watch at a time that suits them?

(These would have been really useful to me. Going to sulk now…)

I’ve been trying to sort out a log-in for the Sysmex training academy. I’ve got one now… Hopefully I can get some CPD from this…
